More than 10 percent of adults living in Jefferson, Lewis and St. Lawrence counties have been diagnosed with diabetes, and many more are living with diabetes or prediabetes but do not know it.

More than 33 percent of adults in the same region have at least one of the following chronic diseases -- prediabetes, diabetes, high blood pressure, heart disease or COPD.

In many cases, these chronic conditions are preventable. In all cases, they can be managed and/or treated, allowing the individual to live a healthy, fulfilling life. The three programs below are designed to help:
